    Abstract
    Introduction: FDG PET/CT is a robust imaging modality to diagnose and stratify prognoses for non-small cell lung carcinoma. However, the role of FDG PET/CT in operable lung adenocarcinoma patients has not been previously investigated in a large cohort with varying pathological stages. The prognostic value of volumetric parameters based on FDG PET/CT was investigated in patients with stage I-III lung adenocarcinoma receiving curative surgery.

    Methods: This retrospective study included 432 patients with lung adenocarcinoma undergoing preoperative FDG PET/CT between January 2016 and December 2017. Clinicopathologic variables, conventional image parameters, such as the maximum standardized uptake value (SUVmax) and mean SUV (SUVmean) of the primary tumor, and volumetric parameters, such as metabolic tumor volume (MTV) and total lesion glycolysis (TLG), were included in Cox regression analysis. Subgroup analysis was conducted to compare hazard ratios (HRs) based on MTV in each pathological stage. A new staging system including volumetric parameters was suggested.

    Results: A total of 432 patients (median age: 62 years; interquartile range: 56-70 years; 225 males) were evaluated. Sex, age, presence of EGFR mutation, pathological stage, MTV, and TLG of the primary tumor were selected as statistically significant prognostic factors for overall survival irrespective of other variables (OS; p < 0.05 for all). Pathological stage, MTV, and TLG of the primary tumor were selected as statistically significant prognostic factors for disease-free survival irrespective of other variables (p < 0.05 for all). The suggested new staging system including MTV as an additional criterion showed better discrimination and prediction for OS than the conventional pathological staging system despite statistical insignificance (concordance index: 0.698 vs. 0.673).

    Conclusions: The volumetric parameters of the primary tumor based on preoperative FDG PET/CT were independent prognostic factors in addition to pathological stage in patients with operable lung adenocarcinoma. The suggested new staging system considering MTV predicted the prognoses better than the conventional pathological staging system.

    FDG PET/CT는 폐암의 진단 및 치료반응 평가, 그리고 예후 예측에 유용하게 사용되는 대표적인 핵의학 영상 검사입니다. 본 연구에서는 근치적 수술을 받은 폐선암 환자들을 대상으로 FDG PET/CT에서 구한 용적 관련 지표의 예후적 가치를 확인하였습니다. 더 나아가 해당 지표들을 기존의 TNM 병기에 반영하여 새롭게 병기를 재구성하였습니다. 기존 TNM 병기도 예후를 잘 나누어주지만(A) 대사 종양 용적을 반영하여 재구성한 병기는 그보다 더 나은 예후 분류 성능을 보였습니다(B). FDG PET/CT에서 구할 수 있는 용적 관련 지표들이 폐암의 예후와 유의미한 상관관계가 있다는 기존 연구들이 있습니다. 그러나 선행 연구들은 대사 특성이 다른 여러 조직 아형을 함께 묶어서 분석하였거나 폐선암 환자들만을 대상으로 분석하였더라도 본 연구보다 적은 숫자를 대상으로 하였습니다. 그리고 최근 TNM 병기가 8판으로 개정되면서 새롭게 병기가 분류되기 시작하였습니다. 이에 본 연구는 8판 TNM 병기에 대해서도 FDG PET/CT가 추가적인 예후적 가치를 지님을 밝혔다는 점에서 의의가 있습니다. 더 나아가 선행 연구들보다 더 많은 수의 폐선암 환자들을 대상으로 하였으므로 FDG PET/CT의 유용성에 대해 더욱 신뢰할 만한 근거를 마련하였다는 점에서 그 의미가 있습니다.
