    Abstract
    Objective: This study evaluates the safety and efficacy of hypofractionated intensity-modulated radiation therapy (IMRT) following radical hysterectomy.

    Methods: This phase II, non-randomized multicenter trial included 61 patients with cervical cancer who were eligible for adjuvant RT after radical hysterectomy. Participants received 40 Gy in 16 fractions using IMRT to whole pelvis. The primary endpoint was the incidence of grade 3 or higher acute gastrointestinal, genitourinary, and hematologic toxicities. Secondary endpoints included patient-reported quality of life and disease-free survival (DFS). Acute toxicities were monitored using the National Cancer Institute's Common Terminology Criteria for Adverse Events v4.0.

    Results: Among the participants, the incidence of grade 3 or higher acute toxicities was 1.6 % (90 % confidence interval, 0-3.7 %). After a median follow-up of 39.5 months, the 3-year DFS rate was 87.1 %, and no grade ≥ 3 late toxicities were reported during the follow-up period.

    Conclusion: Hypofractionated IMRT post-radical hysterectomy demonstrated low severe toxicity rates and effective disease control, suggesting a feasible alternative to conventional fractionation. However, further randomized comparative trials are required to validate the results of this study.

    자궁경부암의 수술후 방사선치료 시행 시 소분할 세기조절방사선치료의 안전성과 유효성을 검증하는 다기관 2상 연구입니다. 보통 골반 부위에 방사선치료 시 소분할 치료를 적용할 경우 부작용 측면에서 우려되는 부분이 있어서 일반적으로는 5-6주 가량의 통상분할 방사선치료를 하게 되는데, 본 연구처럼 소분할 방사선치료로도 치료 효과와 안전성을 담보할 수 있다면, 치료횟수 단축을 통해 환자의 편의성이 크게 증대될 것으로 기대 됩니다. 향후 3상 연구를 포함한 더 많은 결과들이 나오기를 기대합니다.

    하이포프랙셔네이션(hypofractionation)은 각 방사선 조사 횟수당 선량을 증가시키고, 전체 치료 기간은 단축하는 방식을 의미하며 POHIM-RT 연구는 방사선 치료 기간을 단축하면서도 치료 효과와 부작용 간 균형을 평가한 연구로 임상적으로 중요하며 흥미로운 연구로 생각됨
